For Buyers:

1. Forms for new residents are found under the Documents section. These should be completed at closing and submitted to the Board at the mailing address listed on the forms.
• Occupant Information Sheet.
• Authorization Agreement for Preauthorized Payments Form for monthly HOA dues.
• Compliance Statement for SOR Registry.
• Dues and Startup Association Fee Acknowledgement.
2. Please allow 48 to 72 hours for a response after submitting the (above) completed forms.
3. HOA Start Up Fee: See Resident Forms for current information.
4. Monthly HOA Dues: See Resident Forms for current information.
5. Recorded documents of Pheasant Trail Fourth - A Condominium can be obtained either under the Documents section or obtained on the Linn County Recorder's website.

For Sellers:

The following stay with the property:
1. Notebook for Pheasant Trail 4th Owners Association which has Declaration of Submission of Property to Horizontal Property Regime and Amendments, By-laws and Restrictive Covenants.
2. Trash can and recycle bin.
3. Mailbox key.

The following can be found under the Documents section:

• Adopted Rules and Policies of the Pheasant Trail Fourth Owners Association.
• Board meeting minutes.
• Quarterly newsletter.
• Annual budget.
